Changes in CareWeb DC Navigator

There are changes to the Discharge Reports and the Medication Reconciliation steps of the Discharge Navigator, effective Nov 28 2012.

The following message was sent out by MCIT on Wednesday, repeated here in case you missed it:

Effective Wednesday, November 28th:

Discharge Navigator (step 12) Discharge Reports:  
• All medications marked as ‘Entered in Error’ during the medication reconciliation process will NO longer be displayed on the Discharge Summary report.  This change will now align with the patient’s medication list on the Patient Education (step 11), which also does not display medications marked as ‘Entered in Error’.

Discharge Navigator (step 4) Medication Reconciliation:
• Providers modifying a MiChart medication will NO longer need to manually search for that medication.  When the MiChart medication is coded, CareWeb will now auto select and display that medication, along with the appropriate Sig information on the Add Discharge Medication screen.
