Prescription Drug Plan for At-home COVID-19 Tests

Use your Prescription Drug Plan card to get up to eight monthly tests.

U-M health care plan members are now eligible to receive up to eight FDA-approved, at-home COVID-19 tests each month. There is no out-of-pocket cost to the member or eligible dependents as long as the tests are purchased at network pharmacies.

This measure is part of a federal mandate to fight the surge in COVID-19 infections nationwide.

How to Get Your Tests

To ensure you have no cost or reimbursement forms to submit when buying tests, you should:

If you purchase tests at non-network pharmacies or retail stores (such as Amazon), you must submit a claim form and original receipt for reimbursement, which will be capped at $12 per test. View the form and News and Updates for details.

FDA-approved tests include:

  • BD Veritor System SARS-Cov-2
  • BinaxNOW COVID-19 Ag Self Test
  • CareStart COVID-19 Ag Home Test
  • Ellume COVID-19 Home Test
  • FlowFlex COVID-19 Ag Home Test
  • ihealth COVID-19 Ag Rapid Test
  • InteliSwab COVID-19 Rapid Test
  • QuickVue At-Home COVID-19 Test
  • Sofia SARS Antigen Fia Test

Q: What if I need more than 8 tests?

You can purchase as many tests as needed, but U-M will provide reimbursement for only up to 8 tests per covered individual in a 30-day period. Your Health Care Flexible Spending Account can be used for purchases only when not already covered or reimbursed by the U-M Health Plan.

Q. I showed my prescription drug card at the pharmacy, but was told I had to pay up front for my tests. Why?

Call before you go to the pharmacy so you'll know how it's handling the benefit. Some pharmacies are requiring members to pay at the time of purchasing tests, then file for reimbursement. U-M Benefits, its pharmacy benefit manager, and pharmacy networks are working daily to help members receive their full benefit.

If you presented your card at a pharmacy, but were required to pay up front, here's how to file for reimbursement: Complete a prescription drug claim form, include the original receipt, then send both to Magellan Rx, U-M’s prescription drug vendor.
