Dr. Runge, Dr. Miller, and Dr Weinstein

We, the U of M House Officers Association board, along with the undersigned, write to you regarding the impacts of the change made on September 25, 2024, to Michigan Medicine’s IT system limiting access to employee emails. While we understand that this change results from a sincere desire to protect patient information, a goal we share, there are unintended consequences to this change that negatively impact patient care.

The large percentage of patient care that at least partially involves providers who are not on-site at the main medical campus does not seem to have been considered when implementing this change.

Here is a non-exhaustive list of patient care disruptions:

1. Windows Virtual Desktop is notoriously buggy and slow, impacting quick communications within and between physicians and care teams in a way that noticeably and negatively impacts high-quality delivery of care.
2. Employees rotating at the VA have even more limited access to their work email and, thus, other physicians and care teams. These limits put the care of our Veterans at risk.
3. Employees rotating at any partner sites with Trinity Health System lack any ability to log into their work emails. The consequences of this lapse in email access  on patient care are unacceptable.
4. The majority of House Officers lack access to a CoreImage laptop. Windows Virtual Desktop requires a significant amount of the client computer's resources. House Officers may not have access to a personal laptop with the specifications required to facilitate meaningful and efficient use of Windows Virtual Desktop.
5. The HOA has received reports from House Officers who have struggled to connect to email via Virtual Desktop remotely despite adequate technology resources.

It is clear that — while this change was made with the best of intentions — the actual user experience of physicians and other providers was not adequately taken into consideration. This change has already had, and will continue to have, unacceptably negative impacts on patient care and our tripartite mission. Until these issues are addressed, we ask that the hospital roll back email access to the state it was on September 20, 2024. We ask that the hospital address our concerns before making any permanent changes. Any action taken to address our continued cybersecurity must not compromise the mission and work performed by trainees, faculty, and other care team members to deliver the highest level of patient care, medical education, and academic productivity.
